This 4-day advanced-level practitioner course brings together three powerful root cause analysis (RCA) tools – blending barrier-based and traditional approaches for a well-rounded investigative skill set.

Participants will gain hands-on knowledge of:

• Tripod Beta (barrier-based)

• BSCAT (Barrier-based Systematic Cause Analysis Technique)

• TOP-SET RCA (a classical non-barrier-based methodology widely used across industries)

Through this course, participants will be able to:

• Understand the unique strengths and limitations of each RCA method

• Compare and contrast techniques using real-world scenarios

• Apply the most context-appropriate tool based on the complexity and nature of the incident

• Integrate modern safety thinking into traditional investigation models
